Discharge from hospital after liver transplant

Liver transplant information >>>

Leaving Hospital

Once you leave hospital, your transplant co-ordinator will be your main contact with your transplant team. Your transplant co-ordinator will be able to answer many of your questions and put you in touch with other team members who can provide any other services that you may need.

The next section aims to answer some of the questions you may have following your transplant. It also has tips on recognising problems that may necessitate immediate medical attention

Clinic visits

Once you have been discharged from hospital you will be seen in the liver transplant clinic (clinic 5) on Thursday mornings. We will see you weekly initially, in order to check your progress and detect complications as early as possible. The frequency of these visits will gradually be reduced and we usually aim to share your follow up care with your local hospital physician so that your travel to hospital will be minimised. After the first year, if you have not had complications , the risk of developing problems with your new liver is very uncommon.

When you attend the liver transplant clinic, please remember the following

Following your clinic visit, if any alteration is required to your medication or treatment you will be contacted by telephone or letter
